I've had a reply from Steve, and it seems that I'd uncovered a bug in 
PrintPDF 0.60, which was already know to Steve and which does just 
what was described, when "Preprocess file" was ticked. He released 
0.61 five days ago, and this behaves exactly as it should.

Apologies to Aaron, for my suspicions about HostFS, and to everyone 
else for my having started a wild goose chase!
